Form 8941 should be mailed to the address provided on the form. The form is available for inspection and copies are available for a fee of 20 from any Taxpayer Service Center. The Department will send the Form 8941 to the taxpayer(s) in the case where the loss or theft of his/her identity information is determined to be within the first 180 of the applicable period of time (see paragraph (4) above under the heading “For Loss of Identity Information) to determine whether the amount of the loss or theft reported on such form is deductible by the taxpayer.”  Form 8941 must be attached to the taxpayer's income tax return, and must be mailed to the address provided on the form. “For the Loss of Identity Information” means that the taxpayer's identity information was reported (i) as fraud by another person, or (ii) on a report or statement of other person where the loss or theft is the result of fraud by a person under the applicable statute pertaining to identity theft or any other theft.
